Services Contract

This agreement dated________________, between On Site Beauty Works (OSBW) with the principal address of 130 Pearl Street, Ste. 1007, Denver, CO. 80203 sets forth that OSBW is an independent contractor utilizing the facilities of________________________________(address/name of event) i.e. for the purpose of providing _____________________( hair and makeup services - or Specialty Services as indicated).

Terms:
OSBW will collect $100.00 deposit to hold the date and arrange for staff availability. This deposit is non-refundable.
There must be at least four people for the Bridal packages. If for any reason there are less than four people because a guest does not show for any reason, or changes their mind about getting services, full price of the original services will be charged. No refunds are given or reduction in prices. In other words, a bridal service consists of bridal hair and makeup. If one chooses to receive a hairstyle only, the full price of the package will be charged.
Specialty Services: Massage, Facial, Health Coaching will be determined as required by client and fees will be charged accordingly.
OSBW will set fees for services rendered.
This agreement is in effect from _____________________ to __________________. Either party may immediately terminate the agreement, given reasonable cause, as outlined below:

OSBW will:
1. Act in a professional manner, and dress in a professional manner.
2. Set up is done 15 minutes to one half hour prior to event, this to be discussed with host prior to event.
3. OSBW will provide all the necessary items for the services we provide.
4. Provide proof of professional liability insurance and business license.
5. Maintain confidentiality of all client information.

Client will:
1. Provide use of facility (including a safe, private work area), it’s utilities and it’s services, including electrical outlet.
2. Turn in reservation form and contract in a timely manner, read and agree to its terms.

The parties agree that any unresolved disputes about the terms or enforcement of this agreement shall be resolved through arbitration. The non-prevailing party shall be responsible for paying all arbitration costs, unless the arbitration finds partially for both parties, in which all the parties shall each be responsible for half the costs of arbitration.

This constitutes the entire agreement between the parties and replaces any and all prior written or verbal agreements. Should any part of this agreement be considered unenforceable by a court of competent jurisdiction, the remainder of the agreement remains in force. This agreement is governed by the laws of the state of Colorado.
